Overview

This Community Health Center provides quality, cost-effective healthcare to minority communities, the uninsured, and underinsured of Pilsen Little Village, including Primary Care, Behavioral Health, Dentistry, OB/GYN and Midwifery, Pediatrics, Pharmacy, and In-House Lab Services. 

The Family Nurse Practitioner will:

  • diagnose, treat and help prevent mental disorders in a community health center setting
  • provide continuity of care to each patient
  • collaborate with Medical providers 

Schedule: 36 clinic hours per week, 5 days per week including one evening and every other Saturday

Job Requirements

Candidate MUST be bilingual in Spanish or Chinese.
