Overview

We are seeking a Dentist, DDS or DMD for two FQHC locations in Chicago.  Our oral health program has two dental clinics and one school-based dental clinic. We focus on medical-dental integration. At our two dental sites, we have a community farm at one and a birth center at the other.

We were founded as a Federally Qualified Health Center (FQHC) in 1999 have origins going back to 1887 with St. Elizabeth’s Hospital and 1894 with St. Mary of Nazareth Hospital in Chicago.  Our Mission is to promote wellness and provide accessible, compassionate and culturally respectful health care in partnership with our patients and the communities we serve. Our Values:  We provide care for the whole family. Job Requirements

Two years of experience required.

Board certification is required or if board certification eligible must be board certified within one year from start date.

Bilingual English/Spanish is preferred.

40 hours per week.

Call Schedule - no call coverage

Phone:  All providers receive a $50.00 monthly telephone stipend.

Patient Template Schedule

  • 20 to 24 patients are scheduled per day for each dental provider.
  • Providers are scheduled for one evening per week.
  • All providers at the site cover one Saturday every four weeks working from 9am to 2pm and are scheduled another day off during the week.

Electronic Medical Record – Athena. All patient EMR encounters must be closed within 72 business hours.
