This organization is a hybrid FQHC which is provided by and integrated with the Whiteside County Health Department (WCHD). 

Join our comprehensive, community-focused health center where a wide range of services are conveniently available all in one location. Our center offers Optometry, Dental, Oral Surgery, Family Planning, WIC, Immunizations, and Behavioral Health services, allowing our healthcare team to collaborate across multiple areas of care and connect patients with the services they need. This position provides an opportunity to practice in a supportive, multidisciplinary environment while making a meaningful difference in the health and well-being of the community we serve.

20-35 hours per week

Clinic Hours- 8:00-4:30 Monday-Friday

Job Requirements

Job Requirements: To improve the health of Whiteside County and surrounding areas by assessing families avoid communicable diseases and preventable causes of disease and illness through the administering of preventative and acute care services, immunizations, family planning, case management, and counseling; To act as an independent healthcare provider working collaboratively within a multidisciplinary health team; To be responsible for providing comprehensive care to both well and ill patients; To educate patients and/or their families in order to promote wellness, prevent health problems, prevent unintended pregnancies, maintain current health, and intervene in acute or chronic illness.

Language Skills Required:: Spanish Fluency is a plus
