This Community Health Center is comprised of medical professionals who are driven to provide quality care and service to residents of Macon and surrounding counties. Our providers are passionate about bringing the best care to those with the highest need.
From primary care to behavioral health, this CHC provides a wide range of services for infants, young children, teenagers, adults, and seniors. In addition to obstetric and gynecologic care, dermatologic and podiatric services are offered through collaborations with local healthcare partners. Laboratory services, x-ray, and pharmacy are available on site, as well as access to the mammography mobile unit.
Since 1972, we have been operating as a non-profit organization and striving to make our community a healthier one. We continue to expand our services to meet the needs of our community. Today we employ more than 100 people and treat over 19,000 people each year.
Job Summary:
The Advanced Practice Provider is a primary care provider who manages uncomplicated acute and stable chronic illness. The APP is responsible for health assessment, early detection of illness, health promotion, disease prevention and management. May also develop/coordinate special programs related to area of expertise.
Duties/Responsibilities:
- Performs comprehensive health assessment through patient/family history taking, physical examination and assessment of response to actual or potential health problems
- Orders and interprets tests and diagnostic studies as indicated and within patient care guidelines and standards
- Establishes diagnosis based on assessment and appropriate diagnostic tests and procedures
- Implements treatment plans within established medical protocols and determines plan for reassessment
- Prescribes and regulates medications as per collaborative agreement
- Coordinates follow up contacts with patients as needed in regard to abnormal test results, need for additional treatment, referral to specialist(s), etc.
- Determines individual patient and family educational and informational needs and provides/coordinates knowledge acquisition Includes relevant health promotion, prevention and maintenance in health instruction
- Records all assessments, observations, treatment plans, interventions and evaluation in the medical records Manages the care of patients in collaboration with other health professionals and members of the healthcare team in order to make decisions about treatment and attain health goals
- Completes medical record documentation and billing requirements in a timely manner as per organizational standards
- Delegates to clinical staff assessments, treatments or tasks appropriate to licensure, competency and knowledge
- Establishes and maintains appropriate clinical privileges Provides care at offsite locations as assigned
- Collaborates with the health team and community agencies in providing comprehensive care
- Maintains and enhances a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure
- Assumes on call responsibilities mutually agreed upon with the Chief Medical Officer (CMO)
- Accesses resources for indigent, uninsured, disabled or medical underinsured patients and families from federal, state, or local programs
- Regularly attends and participates in provider staff meetings
- Participates on Continuous Quality Improvement committees as assigned by the CMO
- Performs other duties as required within the limits of advanced practice licensure
- 20-40 hours per week

Education and Experience:
- Completion of a required Nurse Practitioner or Physician Assistant Program leading to certification.
- Currently licensed to practice in the State of Illinois
- Extensive knowledge of physical assessment, diagnosis, pathophysiology, pharmacology and management of acute and chronic illness
- Able to work with patients from a variety of socioeconomic and cultural backgrounds
- Prefer a minimum of three years current clinical experience, one year in area of specialty
Physical Requirements:
- Moderate activity including incidental walking, standing, bending, reaching and lifting or carrying for a short distance loads up to 25 lbs.
- Ability to work in conditions that include exposure to blood, bodily fluids and tissues, contagious diseases and/or radioactive or other potentially hazardous materials
- Ability to stand for extended periods of time, eye/hand coordination and manual dexterity necessary to deliver medical services and use equipment
