Overview

Key Responsibilities

  • Provide individual and group counseling services to address mental health diagnoses and life-functioning concerns
  • Develop, implement, and regularly review individualized treatment plans, including type, frequency, intensity, and duration of therapy
  • Evaluate the effectiveness of treatment interventions and modify diagnoses and treatment plans as clinically indicated
  • Conduct comprehensive behavioral health assessments using interviews, screening tools, clinical records, and consultation with medical providers
  • Implement evidence-based therapeutic modalities such as CBT, DBT, Motivational Interviewing, and other appropriate approaches
  • Obtain and utilize medical, psychological, social, and family histories to inform treatment planning
  • Educate patients on mental health conditions, symptoms, coping strategies, and available community support.
  • Track clinical symptoms over time using established outcome measures and adjust treatment when progress is limited
  • Refer and coordinate care with internal services and community-based resources, including substance use treatment and social support services
  • Follow up on referrals and collaborate with external providers to support continuity of care
  • Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR) in compliance with FQHC, billing, and regulatory standards
  • Participate in clinical team meetings, case conferences, quality improvement activities, and required training
  • Practice within scope of licensure and privileging requirements and participate in supervision and performance review processes
  • Perform other duties as assigned

Job Requirements

Minimum Qualifications

  • Master’s degree in social work or counseling from an accredited college or university
  • Current Illinois licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC)
  • Previous clinical experience including crisis intervention, brief assessments, and individual psychotherapy
  • 3 years' experience required
