Overview

This is a professional, direct service position which utilizes an integrated care model of treatment. The Mental Health Provider is located within the primary care medical unit and provides behavioral health treatment to patients as part of the primary care health team. The Mental Health Provider works across the age range to deliver a broad array of warm hand-off and scheduled services including assessment, therapy (brief and long-term), crisis intervention, and coordination. The Mental Health Provider provides consultation services to primary care providers and collaborates with community organizations to ensure whole-person, patient-centered care.

Job Requirements

Job Requirements:

• Master of Social Work, Counseling, Psychology or Human Service-related field
• Active Illinois License (Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, Licensed Marriage and Family Therapist, Licensed Clinical Psychologist)
• Experience in mental health clinical service delivery
• Previous work experience in multicultural setting preferred
