This Community Health Center seeks an Behavioral Health Consultant to deliver behavioral health and wellbeing services to our patients, with a goal of working with patients to support them in reaching their desired goals

This Community Health Center began in 1980 as the Parent Child Center, a three-room clinic at West Suburban Medical Center that offered prenatal, postpartum, and infant care for underserved residents of Chicago’s Austin community. Since then, we have grown to encompass thirteen health centers, serving West Side Chicago and the near west suburbs, as well as providing inpatient care at West Suburban Medical Center and Norwegian American Hospital.

The Behavioral Health Consultant will:

Become an integral part of our primary care team as a behavioral health therapist integrated into the primary care clinic! The Primary Care Behavioral Health (PCBH) model is about providing access to care at the time clients need it and coordinating care with medical providers to achieve the best outcomes.

As a licensed Senior Therapist in one of our Federally Qualified Health Centers (FQHCs), you will be working on-site collaboratively alongside our medical team to provide integrated care to our patients. As a go-to on-site resource for mental health, you will provide evaluations, psycho-social education, just-in-time consultations, mentoring, and treatment services to our clients. Additionally, you will have the opportunity to join the physician in the exam room to discuss with clients how to improve their well-being and quality of life. This role assists clients in developing and accomplishing treatment goals in individual and group settings. It also provides consultation and education to community groups and emergency services. Additionally, this role works competently with clients with coexisting disorders.

To be successful in this role, we are looking for someone who:

  • Competently works with clients with coexisting disorders (youth and adults)
  • Collaboratively works alongside our physicians, nurses, and medical assistants on-site in our health centers.
  • Works with clients on short-term, goal-focused interventions.
  • Is comfortable working in a fast-paced, high-volume environment.

Scheduled Hours: 40 hours a week In Clinic

  • Monday - Friday (8:30 am - 5:00 pm) with 1 evening (10:30 am - 7:00 pm)

Essential Functions

  • Provides crisis intervention services to stabilize and assess the need for and, if possible, deflect a client from psychiatric hospitalization.
  • Secures and coordinates mental health services for the client between various providers, and the client or the client’s caretaker.
  • Facilitates clinical consultation as assigned.
  • Serve as a backup for problem-solving and decision-making in the absence of program managers.
  • Ensures effective communication among all staff regarding administrative and clinical issues.
  • Recommends to the Program Coordinator the transfer of clients from programs in which they are unable or unwilling to function adequately.
  • Performs initial evaluations and assessments of potential clients regarding their appropriateness for participation in the program.
  • Focuses on client empowerment, evidence-based or adopted best practices in providing treatment, seeking to assist clients to build strengths to meet their treatment needs.
  • Facilitates with clients and collaborates with the treatment team to develop, review, and update treatment goals and referrals both internally and externally.
  • Organizes and participates in treatment-related clinical conferences and staffings on assigned clients.
  • Provide education and direction to clients, family members, and/or significant others.
  • Takes responsibility for training in and documenting clinical chart requirements to meet program and state standards.

This FQHC is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) or our School Based clinics in Marion and Carterville Medical Center. This position reports to the Behavioral Health Program Coordinator. The LCSW/LCPC will have a variety of responsibilities focusing on behavioral health assessment and intervention addressing mental health concerns, substance use, and chronic health needs. Responsibilities also include participation in approved training, periodic meetings and peer review.

Responsibilities

  • Represents Shawnee Health in a positive manner with prospective, former and current patients, clients, their families and the community.

  • Accepts same day referrals, Warm Hand Off’s (WHO) from the FQHC's medical providers.

  • Utilizes process and tools for SBIRT’s, PHQ-9, GAD 7, The DAST and the AUDIT and or other validated screening tools.
  • Provides, brief targeted interventions focused on prevention, treatment and recovery services for patients with physical or emotional health needs as well as longer term treatment and support for patients without other options for persistent behavioral health illness. Creates and implements integrated care plans, ensuring access to an array of services relevant to the culture of the patient and the exchange of information among patients, their family and providers. Provides a range of brief, focused prevention and support services for patients with persistent illness. Participates as an individual provider and as a participant on an inter-professional team.
  • Appropriately refers patients with social, economic, environmental and or advocacy needs to an FQHC Case Manager for follow-up.
  • Appropriately refers patients with substance abuse disorders to a substance abuse provider when substance abuse is indicated.
  • Competently uses information technology to support and improve integrated healthcare.
  • Develops and conducts group programs on topics relevant to behavioral health issues and chronic disease.
  • Seeks consultation from BHS III staff, medical providers and or Behavioral Health Program Director as needed.
  • Provides supervision for behavioral health interns as assigned.

Behavioral Health Provider

We are seeking a Behavioral Health Provider to join our comprehensive primary care team. As a central member of the team, you will work closely with Primary Care Providers, MAs, PSRs, Care Navigators, Care Managers, and RNs to provide excellent care to our patients.

Responsibilities:

  • Conduct Screening, Brief Interventions, and Referral to Treatment (SBIRT) for patients presenting to medical appointments
  • Respond promptly to warm hand-offs for patients with Behavioral Health Concerns
  • Regularly consult with Primary Care Team members regarding health status and progress in treatment
  • Provide updated behavioral health diagnoses (mental health and substance use) to Primary Care Team members
  • Collaborate with Primary Care Team members to develop care plans for patients with behavioral health concerns

Therapist, Counselor

This FQHC is seeking a Master’s level clinician responsible for delivering high-quality, culturally competent, person-centered behavioral health services. This includes emergency mental health assessments, crisis intervention, stabilization services, brief therapy, and care coordination for adults experiencing mental health or co-occurring disorders. The successful candidate will operate both in office and community-based settings to ensure access to services in the least restrictive environment, promote recovery, and link individuals to needed resources. This position operates on one of two 12-hour shifts (7:00 AM–7:00 PM or 7:00 PM–7:00 AM) and requires rotational weekend and on-call coverage.

JOB DUTIES

  • Provide immediate mental health crisis intervention, stabilization, and linkage services in response to community and site-based referrals.
  • Conduct comprehensive psychosocial assessments (e.g., IM+CANS, LOCUS) to evaluate mental and physical health status and service needs.
  • Establish provisional DSM-5 diagnoses based on assessment results. Develop individualized service plans that reflect person-centered goals and support recovery-oriented outcomes.
  • Deliver individual, group, and family therapy using evidence-based modalities (e.g., CBT, DBT, MI) as clinically appropriate.
  • Participate in Mobile Crisis Response by responding to crisis calls in the community within 45–60 minutes of dispatch.
  • Work collaboratively with interdisciplinary teams including psychiatrists, nurses, case managers, outreach workers, and external partners (e.g., CPD, DCFS, hospitals).
  • Provide clinical documentation in compliance with Rule 132, Medicaid guidelines, and internal agency policy.
  • Assist with hospitalization procedures when clinically indicated and ensure linkage to outpatient care upon discharge.
  • Advocate for client rights, coordinate referrals and transitions of care, and help bridge service gaps through collaboration with community partners.
  • Participate in required supervision, meetings, clinical case reviews, and ongoing professional development.
  • Work rotational weekends and on-call shifts as scheduled.
