The provider assesses, diagnoses, treats and evaluates the care of patients and their families across the life span. Provides primary health care services emphasizing health promotion and disease prevention, as well as the diagnosis and management of acute and chronic health problems, within the scope of practice for their area of specialty and training. Provides comprehensive medical services for members of family, regardless of age or sex, on continuing basis by performing the following duties.
- Nurse Practitioner will seek direction from a collaborating physician each time he/she needs input when caring for a patient.
- Perform physical examinations and preventive health measures
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs
- Records physical findings, and formulates plan and prognosis, based on patient's clinical problems and health care needs
- Administers or prescribes treatments and medications
- Prepare comprehensive patient care plan as needed
- Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures
- Refer patients to specialist for consultation or to specialized health resources for treatment as applicable
- Performs the following in a timely manner:
- Completes documentation of patient care in the medical records/encounter documentation
- Locks encounters
- Submits codes to billing
- Participates as a member of the Care Team for specific patients following the concept of Patient Centered Medical Home (PCMH).
- Participates in the review of the patient’s medical records prior to a scheduled appointment, to identify if there are any actions needed to improve the patient’s health as a whole during that office visit. (Example: Due for a standardized screening test or procedure, due for an immunization, due for a follow-up lab, etc.)

We are seeking an experienced (2 years minimum) Advanced Practice Clinician to provide care across the lifespan. This Community Health Center began as a volunteer-staffed health center, created in the 1960's to address the issue of high infant mortality in the African-American community. It has since transformed into a health center with staff physicians dedicated to providing primary health care to the underserved in our community. In 2006, the CHC moved to its current location on Bloomington Road in Champaign. In 2011, a Dental Center opened in the same location.
This organization provides over 40,000 patient visits and cares for over 11,500 patients annually. - Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment, provides education and referral (as indicated); and evaluates effectiveness of treatment, education and/or referral.
- Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral source when indicated.
- Orders, may perform and interprets the results of laboratory tests as part of the physical examination and health management. Reports communicable disease in a timely manner to the appropriate Health Dept. per applicable procedures and guidelines.
- Conception and contraception management per applicable protocols and state and federal family planning program guidelines for women presenting for family planning services. This partially includes as indicated: confidential family planning services for adolescents; pregnancy testing and options counseling; provision of emergency contraception; and consideration of all FDA approved contraceptive methods for women seeking contraception.
- Conducts health teaching and counseling during the physical exam, as appropriate, enabling the client to make informed choices about health and treatment options.
- Completes client clinical record documentation in a timely manner specific to the clinical services provided, according to applicable policies and procedures.
- Applies exposure control procedures as described in the FQHC's Infection control policy and Procedure Manual.
- Understands the medical emergency procedures and is familiar with the location of the emergency kit(s)/supplies in all clinic settings.
- Maintains professionalism and adheres to client confidentiality.

POSITION SUMMARY:
The Nurse Practitioner provides primary care services to children and/or adults as part of the provider team at Crusader Community Health collaborating with licensed MD/DO physicians.
ESSENTIAL FUNCTIONS: This will include but is not limited to:
- Accountable for demonstrating the qualities outlined in the “Customer Service Standards of Excellence” program when interacting with patients, their families and friends, and fellow employees.
