This FQHC opened its doors in 1989 as a bilingual, bicultural, nonprofit community health center, located in the heart of the Pilsen community. This organization operates as a FQHC through outpatient clinics with one urgent care location, three traditional outpatient clinics and three school-based clinics. Their services range from Adult & Family Medicine, Behavioral & Mental Health, Dentistry, Diabetes, Family Planning, OB/GYN & Midwifery, Pediatrics, Pharmacy, and Walk In Wellness Center.
The Nurse Practitioner provides primary health care to specified patient populations of the FQHC. The Nurse Practitioner utilizes theories of psychosocial, developmental, educational and nursing sciences to facilitate the delivery of this health care. The Nurse Practitioner works inter-dependently with and independently of the multi-disciplinary team.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Based on protocols, standing orders, or in collaboration with a physician, arranges for necessary laboratory, radiology and/or other diagnostic tests to complete delivery of primary health care

The Family Nurse Practitioner or Physician Assistant works under the direct supervision of the Chief Medical/Population Health Officer, who is also the collaborating physician. FNP/PA completes all the duties associated with their clinical license and manages patient care in accordance with the professional guidelines of the FQHC. This position is unique as it is a collaborative position between the FQHC in LaGrange and in Summit. The objective is to optimize health for those adults experiencing housing insecurity by improving access to primary care FNP/PA will spend 50% of their time providing medical assessment and care in a clinical location at the Summit, which is a dedicated location for individuals experiencing homelessness, and 50% at the FQHC in La Grange.
Essential Duties and Responsibilities:
Patient Care
- Provides general medical care and treatment to patients under the direction of a physician and as dictated by their Illinois Clinical License and PCH policies and procedures
- Provides primary patient health assessment.
- Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physicians to diagnose and treat acute, chronic, and long-term health issues of patients.
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs.
- Prescribes or recommends medication or other forms of medical treatment such as therapeutic procedures.
Case Management
- May refer patients to physicians for consultation or to specialized health resources for treatment.
- Coordinates patient treatment, management and planning with the clinical staff and physician to provide continuity of care, including assistance with referrals to other agencies and health care providers.
- Submits health care plan and goals of individual patients for periodic review and evaluation by physician.
- Provides telephone support to patients, physicians hospitals, pharmacies, and other health care providers.
Patient Records
- Responsible for documentation of patient care; records physical findings, and formulates plan and prognosis, based on patient’s condition.

Patient Education

This Community Health Center began as a volunteer-staffed health center, created in the 1960's to address the issue of high infant mortality in the African-American community. - Assesses and interprets medical and psycho-social histories; performs physical examinations (including gynecological and breast examination as appropriate); identifies problems; formulates a plan of treatment, provides education and referral (as indicated); and evaluates effectiveness of treatment, education and/or referral.
- Consults with or refers clients with abnormal conditions or other medical, psychological or social problems to the collaborating physician or other appropriate referral source when indicated.
- Orders, may perform and interprets the results of laboratory tests as part of the physical examination and health management. Reports communicable disease in a timely manner to the appropriate Health Dept. per applicable procedures and guidelines.
- Conception and contraception management per applicable protocols and state and federal family planning program guidelines for women presenting for family planning services. This partially includes as indicated: confidential family planning services for adolescents; pregnancy testing and options counseling; provision of emergency contraception; and consideration of all FDA approved contraceptive methods for women seeking contraception.
- Conducts health teaching and counseling during the physical exam, as appropriate, enabling the client to make informed choices about health and treatment options.
- Completes client clinical record documentation in a timely manner specific to the clinical services provided, according to applicable policies and procedures.
- Applies exposure control procedures as described in the FQHC's Infection control policy and Procedure Manual.
- Understands the medical emergency procedures and is familiar with the location of the emergency kit(s)/supplies in all clinic settings.
