Job Description The LCSW/LCPC works in conjunction with members of the Care Team to manage psycho-social aspects of chronic and acute diseases, apply behavioral principles to address lifestyle and health risk issues and provide consultation in the treatment of behavioral concerns, mental disorders, substance abuse and psycho- social issues. Bilingual in Spanish or any other language a plus.
Duties also include, but are not limited to:
- Promote and integrate behavioral health services by: Maintaining protocols and procedures for integrated behavioral health activities.
- Assisting the primary care provider in recognizing, treating and managing mental health and psycho-social issues and acts as a contributing member to the primary care team and behavioral health team.
- Conducting client intakes, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care provider concerning the clients’ treatment goals and plan.
- Providing consultation to the primary care providers to enhance patient care.
- Providing brief, focused intervention for clients who are in need of mental health services.
- Assisting in the detection of “at risk” clients and in the development of plans to prevent worsening of their condition.
- Monitoring and coordinating the delivery of health services for clients as related to behavioral health care, including linking with other treatment providers not only within the primary care setting but, with the clients’ permission, outside it as well.
- Documenting the clients’ progress and diagnostic information in the treatment chart.
- Keeping the primary care providers fully informed of the clients’ needs and progress and works with providers to formulate treatment plans.
- Providing consultation to clinic management and other team members about behavioral services and suggested areas of outcome and program evaluation
- Participating in case conferences with appropriate medical and ancillary team members.
- Supporting Quality Assurance activities related to behavioral health care.
- Providing program reports.
- Assists the clients in complying with any medical treatment initiated by the primary care provider, such as offering strategies to cope with medication side effects.
- Functioning as a member of a multi-interdisciplinary team.
- Possessing proficiency in the identification and treatment of mental disorders
- Having the knowledge of evidence-based behavioral assessments and interventions relevant to medical conditions, e.g., disease management; treatment adherence; and lifestyle change.
- Possessing the ability to make quick and accurate clinical assessments.
- Being comfortable with the pace of primary care, working with an interdisciplinary team, and has strong communication skills.
- Possessing care-management skills and knowledge of local resources for outside referrals.
- Possessing skill in targeted, brief psychotherapy and in running group sessions.
- Possessing the ability to document services in a way that is useful both to the primary care provider and to management for quality- improvement services.
- Consultation liaison skills.
- Ability to be flexible with schedules and work in a fast-paced environment.
- Experience and knowledge of multi-ethnic and under-served communities
- Travel to additional clinic locations in Jacksonville and Taylorville may be required.

Duties and Responsibilities:
- Provide an assessment of the client’s mental health and substance abuse needs to develop a service plan.
- Provide supportive counseling and advocacy for clients suffering from co-occurring disorders, including substance abuse.
- Provide DSM-5 diagnoses and the ability to utilize research-based treatment modalities effectively.
- Understanding and practicing professional ethics code of conduct and HIPAA compliance. Maintain up-to-date client records and statistical data.
- Prepare documentation for the client’s file (intake, encounter notes, treatment plan, etc.)
- Report counseling/case management services to the supervisor every week.
- Refer clients for appointments with a psychiatrist as needed.
- Provide referrals for community resources and social services programs within the FQHC service tracks.
- Attend staff meetings and other FQHC activities.
- Participate in staff training, development, and support activities.
- Develop and implement new processes and procedures for services provided.
- Travel to agency locations in Chicago Northside and Addison Township may be required.
- Other duties as assigned that will support FQHC’s mission.

This Community Health Center seeks an Behavioral Health Consultant to deliver behavioral health and wellbeing services to our patients, with a goal of working with patients to support them in reaching their desired goals.
This Community Health Center began in 1980 as the Parent Child Center, a three-room clinic at West Suburban Medical Center that offered prenatal, postpartum, and infant care for underserved residents of Chicago’s Austin community. Since then, we have grown to encompass thirteen health centers, serving West Side Chicago and the near west suburbs, as well as providing inpatient care at West Suburban Medical Center and Norwegian American Hospital.
The Behavioral Health Consultant will:
- provide behavioral health care services to patients in all life cycles in a primary care setting
- participate with a multidisciplinary team to ensure holistic psychosocial and medical needs are met.

Become an integral part of our primary care team as a behavioral health therapist integrated into the primary care clinic! The Primary Care Behavioral Health (PCBH) model is about providing access to care at the time clients need it and coordinating care with medical providers to achieve the best outcomes.
As a licensed Senior Therapist in one of our Federally Qualified Health Centers (FQHCs), you will be working on-site collaboratively alongside our medical team to provide integrated care to our patients. As a go-to on-site resource for mental health, you will provide evaluations, psycho-social education, just-in-time consultations, mentoring, and treatment services to our clients. Additionally, you will have the opportunity to join the physician in the exam room to discuss with clients how to improve their well-being and quality of life. This role assists clients in developing and accomplishing treatment goals in individual and group settings. It also provides consultation and education to community groups and emergency services. Additionally, this role works competently with clients with coexisting disorders.
To be successful in this role, we are looking for someone who:
- Competently works with clients with coexisting disorders (youth and adults)
- Collaboratively works alongside our physicians, nurses, and medical assistants on-site in our health centers.
- Works with clients on short-term, goal-focused interventions.
- Is comfortable working in a fast-paced, high-volume environment.

Essential Functions
- Provides crisis intervention services to stabilize and assess the need for and, if possible, deflect a client from psychiatric hospitalization.
- Secures and coordinates mental health services for the client between various providers, and the client or the client’s caretaker.
- Facilitates clinical consultation as assigned.
- Serve as a backup for problem-solving and decision-making in the absence of program managers.
- Ensures effective communication among all staff regarding administrative and clinical issues.
- Recommends to the Program Coordinator the transfer of clients from programs in which they are unable or unwilling to function adequately.
- Performs initial evaluations and assessments of potential clients regarding their appropriateness for participation in the program.
- Focuses on client empowerment, evidence-based or adopted best practices in providing treatment, seeking to assist clients to build strengths to meet their treatment needs.
- Facilitates with clients and collaborates with the treatment team to develop, review, and update treatment goals and referrals both internally and externally.
- Organizes and participates in treatment-related clinical conferences and staffings on assigned clients.
- Provide education and direction to clients, family members, and/or significant others.
- Takes responsibility for training in and documenting clinical chart requirements to meet program and state standards.

This FQHC is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) or our School Based clinics in Marion and Carterville Medical Center. This position reports to the Behavioral Health Program Coordinator. The LCSW/LCPC will have a variety of responsibilities focusing on behavioral health assessment and intervention addressing mental health concerns, substance use, and chronic health needs. Responsibilities also include participation in approved training, periodic meetings and peer review.
Responsibilities
- Represents Shawnee Health in a positive manner with prospective, former and current patients, clients, their families and the community.

- Handles confidential information with tack and discretion.
- Accepts same day referrals, Warm Hand Off’s (WHO) from the FQHC's medical providers.
- Establishes rapport quickly and communicates effectively with patients, their family members and other providers.
- Conducts brief, evidenced based and developmentally appropriate screenings and conducts or arranges for more detailed Behavioral Health assessment when indicated.
- Utilizes process and tools for SBIRT’s, PHQ-9, GAD 7, The DAST and the AUDIT and or other validated screening tools.
- Provides, brief targeted interventions focused on prevention, treatment and recovery services for patients with physical or emotional health needs as well as longer term treatment and support for patients without other options for persistent behavioral health illness. Creates and implements integrated care plans, ensuring access to an array of services relevant to the culture of the patient and the exchange of information among patients, their family and providers. Provides a range of brief, focused prevention and support services for patients with persistent illness. Participates as an individual provider and as a participant on an inter-professional team.
- Appropriately refers patients with social, economic, environmental and or advocacy needs to an FQHC Case Manager for follow-up.
- Appropriately refers patients with substance abuse disorders to a substance abuse provider when substance abuse is indicated.
- Competently uses information technology to support and improve integrated healthcare.
- Develops and conducts group programs on topics relevant to behavioral health issues and chronic disease.
- Seeks consultation from BHS III staff, medical providers and or Behavioral Health Program Director as needed.
- Provides supervision for behavioral health interns as assigned.
- Performs other duties as assigned.

- Regularly consult with Primary Care Team members regarding health status and progress in treatment
- Provide updated behavioral health diagnoses (mental health and substance use) to Primary Care Team members
- Collaborate with Primary Care Team members to develop care plans for patients with behavioral health concerns
