This Community Health Center seeks a General Dentist to care for the medically underserved of one of the fastest growing counties in the United States.

This Community Health Center has been serving the needs of a growing population since 1946. We provide comprehensive public health programs, behavioral health resources, and primary health care and dental facilities located in Joliet, Bolingbrook, and Monee.

The Dentist will:

• provide high-quality preventive and restorative dental care, including simple and surgical extractions, to both children and adults
• work cohesively with our diverse team of nearly 400 agency professionals

Hours are Wednesday and Friday from 8:00 a.m. - 4:00 p.m. and Saturday from 7:30 - 3:30 p.m. - (22.5 hours per week)

To provide oral health care services to patients of Pillars Community Health within the Scope of Practice and in accordance with the FQHC DDS privileges, under the general supervision of the Dental Director. Participates in and works to achieve Quality outcomes established by the FQHC. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

Direct Provision of Service

• Personally provide care to patients and provide the delivery of competent, accurate dental care and treatment to all patients as assigned and in accordance with the FQHC Health Scope of Practice guidelines, and within practice certifications and agency privileging, and within parameters of Illinois Dental Practice Act.
• Collaborate with all clinic operations functions to ensure ongoing improvements and clinic productivity, patient satisfaction and financial outcomes.
• Directly affect improvements in patient well-being through accurate and timely diagnosis, development of treatment plans and delivery of relevant, high-quality care.
• Provide education to patients and their families about oral health, self-care and dental hygiene.
• Participates in telephone support for patients, including after-hours, as directed by Dental Director.

Patient Records

• Ensure all patient records, charts and all related documentation is maintained current and consistent with best practices in the health care field, in accordance with agency policies and, as well as within all relevant laws and regulations.
• Responsible for documentation and coding of patient care; records findings and formulates and documents treatment plan based on patient’s condition.
• Maintains patient information and confidentiality in accordance with applicable laws and agency policies

Clinical Quality Performance

• Assists in the compilation of statistical data and patient outcomes for agency needs, as requested
• Participates in agency quality performance programs
• Helps to maintain safe and clean patient care environment
• Adheres to OSHA guidelines for safe work environment

This Community Health Center is seeking a Dentist to provide general dental services to children and adults as part of the provider team. This CHC serves the Rock River Valley area with quality primary health care for all people in need across our eight locations. ESSENTIAL FUNCTIONS:

This FQHC is seeking a compassionate Full-Time Dentist to deliver high-quality dental care to individuals experiencing homelessness and participants in the Ryan White Program. 

Essential Duties and Responsibilities:

• Facilitates entry of the client into the oral health care system by assessing the primary oral health care needs, including eliciting comprehensive health histories, performing oral examinations, order and/or performing diagnostic tests, analyzing collected data to determine client health status and formulating a problem list (diagnosis). 
• Provides coordinated, continuous, and culturally competent oral health care to clients. 
• Prescribes a variety of treatment modalities made available to the provider through Health Center’s resources to meet preventative oral health and disease needs of participants.
• Collaborates with physicians and dental auxiliaries to assure that oral health instruction and counseling is provided.
• Manages and evaluates the plan of care, process, and outcome, and then modifies plan as needed.
• Coordinates services in conjunction with other team members to meet client's oral health care needs, and to ensure the continuity of care.
• Collaborate with other multi-disciplinary professionals and community agencies to provide high quality care, including referral to appropriate dental specialists, if indicated.
• Providers are expected to continue follow up with those referred to dental specialist to monitor status of such referrals in the continuum of care for that individual to promote completion of treatment or completion of alternatives to care.
• Adheres to collaborative principles of respect and joint decision making.
• Participates in quality improvement activities though protocol development, staff training and collaborative peer review.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide direct clinical care to a diverse patient panel in an outpatient clinic setting, managing a wide range of health conditions including acute and chronic diseases.
• Perform comprehensive health assessments, including physical exams, diagnostic tests, and the development of treatment plans.
• Prescribe medications, administer immunizations, and monitor ongoing care for patients with acute and chronic health conditions.
