This Community Health Center seeks a Family Medicine Physician to provide a continuum of care that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). The Family Medicine Physician will also supervise and teach family medicine residents in clinical setting, support residency clinic operations, and participate in other family medicine residency activities in collaboration with residency program leadership.

The Family Medicine Physician will:

Our Physicians serve our community by providing comprehensive direct medical care and services and working with other care team members to provide quality patient-centered care to our patients. Practiced specialists in our health centers included OB/GYN, Family Medicine, & Internal Medicine. Salary determined based on qualifications and experience.
Essential Functions
- Works with staff, as a team, to provide high-quality care to our patients by identifying and assessing health concerns, examining the patient based on the health concern, and creating a plan specific to the patient needs.
- Develops and maintains an awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.
- Provides clinical consultation to other staff members and supervision of staff and students as assigned.
- Demonstrates adherence to National Patient Safety Goals and plan.
- Participates in quality improvement activities to assess, monitor and improve program services as well as mandatory in-services and drills, attends staff meetings and other training, including appropriate age-specific educational training offered on a regular basis for professional development.
Scheduled Hours: Based on Clinic Needs

Involving a combination of clinical and academic responsibilities, with a focus on training and supervising residents in the full scope of family medicine, including obstetrics. This position requires a clinician who shares our values and mission, committed to delivering care that addresses the whole person and contributes to improved health outcomes.
As part of our commitment to patient-centered care, the clinician will collaborate with interdisciplinary teams to provide continuous care and contribute to the overall wellness of patients.
Key Responsibilities:
- Provide direct clinical services, including assessment, diagnosis, and treatment of individuals and families in an outpatient setting.
- Collaborate with primary care providers, SIU faculty, and other healthcare professionals including BJC Alton Memorial.
- Supervise and mentor family medicine residents in both outpatient and inpatient settings.
- Participate in 3-4 half day clinical sessions per week, providing direct patient care and resident supervision.
- Participate in the education and training of family medicine residents by providing direct supervision during outpatient rotations, including OB.
- Participate in academic activities, including didactics, meetings, curriculum development and advising including one-half day didactic teaching and 3 half-days dedicated to scholarly work.
- Provide coverage for FM inpatient services managing hospitalized patients at least 8 times per year.
- Maintain accurate and timely documentation of patient care, adhering to all legal and regulatory standards.
- Ensure high levels of proficiency within scope, following the expectations of the Patient-Centered Medical Home model.

This FQHC was established as a federally qualified health center (FQHC) in October 1999. As a FQHC, we receive some federal support to provide primary health care, oral health care and behavioral health care to anyone who enters our doors, without regard to socioeconomic barriers, including ability to pay. CCHC, a locally held, private not-for-profit, is governed by a volunteer Board of Directors that is required to maintain a consumer majority.

Job Description:
- Preventive Care: Provide preventive care services, including routine check-ups, immunizations, and health education to promote overall wellness.
- Coordination of Care: Collaborate with other healthcare professionals to coordinate patient care and refer patients to specialists when necessary.
- Record Keeping: Maintain detailed and accurate medical records of patients' histories, treatments, and progress.
- Patient Education: Educate patients and their families about managing chronic diseases, lifestyle changes, and preventive health measures.

Clinical and Operational Responsibilities
General Clinical
- Obtains health history and perform physical examinations.
- Diagnoses, treats, counsels, and coordinate care for each patient.
- Reviews and update problem list and longitudinal plan of care at every visit.
- Meets documentation standards and completes all progress notes within 24 hours following the patient visit.
- Fulfills on-call responsibilities in accordance with the schedule developed by the FQHC and responds to calls within 20 minutes.
- Prescribe medication in accordance with Illinois statute and professional practice guidelines.
- Performs all duties in conformity with the quality standards and clinical guideline established by the FQHC.
- Meets Meaningful Use standards which is defined as using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, engage patients and families in their health care, improve care coordination, improve population and public health; all the while maintaining privacy and security.
- Manages EHR In-Basket, transfers In-Basket responsibilities to colleagues when appropriate, and responds to messages (e.g. lab results, refills, patient calls, patient messages, co-sign orders, co-sign charts, and open charts) within 72 hours,
Patient Support and Education
- Provides culturally competent care Recognizes patients’ families as an integral part of patients’ care teams.
- Ensures that patients’ and families’ needs are met, and clinic policy is followed through.
- Encourage patients and families to voice questions and/or concerns. Works with patients and their families as appropriate to encourage care self-management
- Conducts patient education activities, including pre and post instruction exams and procedures, instructions related to illness and treatment, and reinforcement of provider’s follow-up instructions
- Utilizes printed health education materials as appropriate and document all instructions given; while giving consideration to an individual varying level of understanding and adjusts level of presentation to ensure the patient is knowledgeable of medical condition and plan of care
- Works with care team and case management to provide for coordinated delivery of case management services.
- Ensures patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately
- Meets intensity of service and/or severity of illness admission criteria
PCMH Requirements
- Coordinates with health center staff at all levels to ensure patient needs are met
- Participates in health center huddles and meets scheduled health center times and productivity standards.
- Assures patients are seen in a timely and clinically appropriate manner and involves health center employees including medical assistants and front desk staff in this process.
- Assures care teams properly carry out effective treatment, education and performs clinical tasks in accordance with acceptable policy and procedure.
Provider Development
- Participates in the orientation and training of staff as needed.
- Ensures care team meets internal certification requirements for patient education and the administration of medication.
- Participates in proficiency testing as required by the FQHC to ensure that proper standards of care are being maintained.
- May function as a preceptor in teaching programs.
- Adheres to Health Resources and Services Administration (HRSA) and Federal Tort Claims Act (FTCA) regulations.

How You’ll Help Patients:
- Provide high quality medical care to the patients in accordance with Specialty Certification and policies and procedures.
- Is fully supportive of all quality initiatives.

- Provides care in age-appropriate fashion according to one’s specialty.
- Report to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
- Engage in all offered training, consultations and supervisions made available.
- Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.
- Follow and fully implement Corporate Compliance Program. Perform other duties as assigned.
