Your Goal: This position is responsible for the provision of high-quality clinical care for the patients of the FQHC.
How You’ll Help Patients:
- Provide high quality medical care to the patients of the FQHC in accordance with Specialty Certification and the FQHC Policies and Procedures.
- Is fully supportive of all quality initiatives. Is fully receptive to other responsibilities as assigned.
- Provides all forms of leadership, supervision, direction, training and role modeling to all supportive staff (eg: NA, MA, LPN, RN, PA, NP). Works to assure that all responsible areas are fully compliant with all applicable federal and state rules, regulations and laws.
- Insures that all responsible areas are in compliance with all FQHC objectives, systems, policies, procedures, directives and contractual agreements as indicated by supervisors, contracts, training personnel and FQHC policies and procedures.
- Maintains a level of efficiency that indicates satisfactory productivity.
- Provides on call coverage and rounding on inpatients (as appropriate).
- Provides care in age-appropriate fashion according to one’s specialty.
- Report to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
- Engage in all offered training, consultations and supervisions made available.
- Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.
- Follow and fully implement Corporate Compliance Program.
- Perform other duties as assigned.

This FQHC opened its doors in 1989 as a bilingual, bicultural, nonprofit community health center, located in the heart of the Pilsen community.
This organization operates as a FQHC through outpatient clinics with one urgent care location, three traditional outpatient clinics and three school-based clinics. Services range from Adult & Family Medicine, Behavioral & Mental Health, Dentistry, Diabetes, Family Planning, OB/GYN & Midwifery, Pediatrics, Pharmacy, and Walk In Wellness Center.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provides patient care in a specified number of clinic sessions per week
- Provides out-patient clinic consultation and in-patient backup for the Nurse-midwifery service, for a specified amount of time each month
- Admits patients to the hospital as necessary and serves as primary attending physician
- Provides comprehensive integrated patient care in cooperation with other programs (i.e., WIC, prenatal classes, nutrition counseling, case management, health education)
- Works cooperatively with other physicians for cross-coverage of patients, both clinic and in-patient when appropriate
- Participates in Quality Assurance activities
- Participates in data collection
- Attends service/staff meetings as appropriate
- Meets with the service director on a timely basis to review policies, procedures and sign protocols
- Works cooperatively and supportively with internship programs
- Performs other related duties as assigned
This position requires compliance with all written standards, including its Standards of Conduct, Joint Commission standards, all policies and procedures and Corporate Compliance requirements. Compliance will be considered as part of the regular performance evaluation.

This Community Health Center has deep roots in Champaign's underserved communities and serves a diverse population. It began as a volunteer-staffed health center, created in the 1960's to address the issue of high infant mortality in the African-American community. This organization provides over 40,000 patient visits and cares for over 11,500 patients annually. Approximately 30% of patients are Spanish speaking. The health center is a family practice clinic and delivers integrated care, with on-site mental health counselors, psychiatrists, and a dental clinic. Patients are supported for social service needs. The health center has a great referring relationship with providers in the community, which provides our patients great access to specialty care.

The Family Medicine Physician provides clinical services and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served, in accordance with the FQHC mission and strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards. As a Family Medicine Physician, provides medical services to individuals and families, emphasizing health promotion and disease prevention, as well as the evaluation and management of acute and chronic diseases, and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Monday - Friday 8:00 a.m. - 4:30 p.m. These hours include lunch and admin time.
Essential Functions
- Provides high quality professional family services to the patients of HHS.
- Provides on-call availability to patients of HHS on weekends, holidays, and all other non-regular hours: the responsibility for alternate physician coverage during vacations and conferences remains with the physician.
- Maintains the consistency and quality of all services provided to HHS and its patients.
- Maintains full, current, and timely medical records of patients, including reports of all examinations, procedures, and other services performed by physician and other support staff.
- Exercises diligence in keeping costs of HHS to a minimum.
- Plans and coordinates comprehensive medicine services with the continuum of care requirements of patients and with other providers of services.
- Participates in the marketing of family medicine services, as requested.
- Coordinates medicine services with members of the health team to provide for the highest quality and most efficient delivery of medicine services including, as acting as a collaborating physician for a midlevel clinician(s) (Nurse Practitioner (NP) and/or Physician Assistant (PA).
- Participates in HHS’s peer review program.
- Participates in the Quality Improvement and Assurance Program of HHS.
- Complies in full with HHS’s Infection and Exposure Control Plan.
- Performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives, as directed by a supervisor. Supports clinic compliance with all applicable federal, state, local, and HHS rules, regulations, protocols, and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health, and confidentiality.
- Supports and is involved in HHS’s continuous quality improvement efforts designed to improve patient outcomes.
- Works in consultation with clinical teams, direct clinical support staff, and indirect clinical support staff to develop and implement policies and procedures that maximize patient-centered communication and services.
- Maintains and assures confidentiality of patient information in accordance with HHS’s policies.
- Reports building/equipment problems through the appropriate channels.
- Performs any clerical duty or department related task as assigned by supervisor in a continuously changing medical practice.
- Attends all staff meetings, department meetings, and any other meetings as required.
