Essential Duties and Responsibilities
- Provide appropriate clinical services including wellness and lifestyle modifications.
- Performs physical examinations of new and existing patients while ordering additional diagnostic and screening tests as appropriate.
- Interprets examination findings and test results, and implements treatment plans
- Prepares and reviews case histories and obtains data through interviews
- Provides continuing, comprehensive health maintenance and medical care to the entire family unit, to include preventive medicine, behavioral sciences and community health
- Determines need for consultation and assists in medical care treatment provided at the direction of other specialists Instructs other health care providers and non-medical personnel in a variety of health related topics, including first-aid measures, basic and advances cardiac life support, and health care maintenance
- Participates in the instruction of medical students and residents who participate in a clinical rotation at FCHC
- Provides care consistent with the quality standards of the organization, participates in peer review activities, and works with administration to reduce risk and maintain patient safety.
- Performs other duties as assigned.

Our healthcare organization is seeking a highly skilled and compassionate Family Medicine Physician to provide exceptional primary care services to underserved communities. As an Family Medicine Physician, you will be responsible for managing patient care in collaboration with other clinical staff and members of the care team.
Responsibilities:
- Provide comprehensive and high-quality primary care services to patients in the office, hospital, or other locations within the community as assigned
- Serve as a consultant to clinical management regarding clinical goals and services

The Family Medicine Physician provides clinical services and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served, in accordance with the FQHC mission and strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards. As a Family Medicine Physician, provides medical services to individuals and families, emphasizing health promotion and disease prevention, as well as the evaluation and management of acute and chronic diseases, and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Monday - Friday 8:00 a.m. - 4:30 p.m. These hours include lunch and admin time.
Essential Functions
- Provides high quality professional family services to the patients of HHS.
- Provides on-call availability to patients of HHS on weekends, holidays, and all other non-regular hours: the responsibility for alternate physician coverage during vacations and conferences remains with the physician.
- Maintains the consistency and quality of all services provided to HHS and its patients.
- Maintains full, current, and timely medical records of patients, including reports of all examinations, procedures, and other services performed by physician and other support staff.
- Exercises diligence in keeping costs of HHS to a minimum.
- Plans and coordinates comprehensive medicine services with the continuum of care requirements of patients and with other providers of services.
- Participates in the marketing of family medicine services, as requested.
- Coordinates medicine services with members of the health team to provide for the highest quality and most efficient delivery of medicine services including, as acting as a collaborating physician for a midlevel clinician(s) (Nurse Practitioner (NP) and/or Physician Assistant (PA).
- Participates in HHS’s peer review program.
- Participates in the Quality Improvement and Assurance Program of HHS.
- Complies in full with HHS’s Infection and Exposure Control Plan.
- Performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives, as directed by a supervisor. Supports clinic compliance with all applicable federal, state, local, and HHS rules, regulations, protocols, and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health, and confidentiality.
- Supports and is involved in HHS’s continuous quality improvement efforts designed to improve patient outcomes.
- Works in consultation with clinical teams, direct clinical support staff, and indirect clinical support staff to develop and implement policies and procedures that maximize patient-centered communication and services.
- Maintains and assures confidentiality of patient information in accordance with HHS’s policies.
- Reports building/equipment problems through the appropriate channels.
- Performs any clerical duty or department related task as assigned by supervisor in a continuously changing medical practice.
- Attends all staff meetings, department meetings, and any other meetings as required.

Position Summary:
The Family Physician directs and oversees the provision of quality care to assigned patients, consistent with guidelines contained within the quality management, risk management, infection control, customer service, and safety programs for the FQHC.
Essential Duties and Responsibilities:
- Must be able to see patients from 0 to the entire lifespan
- Ensures the daily health center patient load is seen promptly and clinically appropriately.
- Ensures that all clinical activities occurring within the health center are conducted according to accepted guidelines for reimbursement.
- Completes all progress notes with date/time immediately following the patient visit.
- Participates in after-hours call rotation and responds to after-hours calls within 20 minutes when on call. *** Currently, this on-call is not active as the FQHC has hired a service to complete this requirement.
- Adheres to approved FQHC treatment protocols.
- Conducts patient education activities, including pre- and post-instruction exams and procedures, instructions related to illness and treatment, such as diet, and reinforcement of the physician’s follow-up instructions.
- Utilizes printed health education materials as appropriate and documents all instructions given.
- Considers the health literacy of patients and tailors education accordingly to ensure patients are knowledgeable about medical conditions and the plan of care.
- Ensures core team staff properly carry out effective treatment and education.
- Ensures that core team staff perform clinical tasks in accordance with accepted policies and procedures.
- Participates in the orientation and training of staff as needed. Ensures core team staff meet internal certification requirements for patient education and the administration of medication.
- Ensures follow-up records, as assigned, to determine conformance with accepted clinical protocols. Ensures incident reports are completed for those occurrences defined as incidents in accordance with policy and procedure.
- Ensures that patients presenting with greater acuity than what is clinically appropriate within the health center are transferred appropriately.
- Ensures the patient’s medical record meets documentation standards.
- Meets scheduled health center times and productivity standards. Monitors and ensures the cost-effective utilization of clinical resources.
- Collaborate with the core team and case management staff to ensure the coordinated delivery of case management services.
- Provides triage services for emergency walk-in patients, identifying those who can't wait to receive care and those who require immediate medical attention, and schedules services within a time frame that meets the patient’s medical needs.
- Effectively demonstrates required competencies in the provision of educational, case management, triage, and medication administration services.
- Adheres to the organization’s mission-based approach “to promote physical and emotional health and psychological well-being of individuals and families by offering hope, health, and healing.”

Involving a combination of clinical and academic responsibilities, with a focus on training and supervising residents in the full scope of family medicine, including obstetrics. This position requires a clinician who shares our values and mission, committed to delivering care that addresses the whole person and contributes to improved health outcomes.
As part of our commitment to patient-centered care, the clinician will collaborate with interdisciplinary teams to provide continuous care and contribute to the overall wellness of patients.
Key Responsibilities:
- Provide direct clinical services, including assessment, diagnosis, and treatment of individuals and families in an outpatient setting.
- Collaborate with primary care providers, SIU faculty, and other healthcare professionals including BJC Alton Memorial.
