Location: Chicago, IL - Specialty: Family Medicine, General

This Community Health Center seeks a Family Medicine Physician to provide a continuum of care that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). The Family Medicine Physician will also supervise and teach family medicine residents in clinical setting, support residency clinic operations, and participate in other family medicine residency activities in collaboration with residency program leadership.

The Family Medicine Physician will:

  • provide outstanding, patient-centered primary and preventive care to patients of all ages
  • work with a team of physicians, nurse practitioners, midwives, and other providers, along with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care

  • exhibit a positive attitude during daily activities and foster a positive work environment for the entire team

Located at our Alton, IL Health Center, this role will provide high-quality, patient-centered care in an outpatient setting, working collaboratively with other healthcare professionals. Involving a combination of clinical and academic responsibilities, with a focus on training and supervising residents in the full scope of family medicine, including obstetrics. This position requires a clinician who shares our values and mission, committed to delivering care that addresses the whole person and contributes to improved health outcomes.

As part of our commitment to patient-centered care, the clinician will collaborate with interdisciplinary teams to provide continuous care and contribute to the overall wellness of patients.

Key Responsibilities:

  • Provide direct clinical services, including assessment, diagnosis, and treatment of individuals and families in an outpatient setting.
  • Collaborate with primary care providers, SIU faculty, and other healthcare professionals including BJC Alton Memorial.
  • Supervise and mentor family medicine residents in both outpatient and inpatient settings.
  • Participate in 3-4 half day clinical sessions per week, providing direct patient care and resident supervision.
  • Participate in the education and training of family medicine residents by providing direct supervision during outpatient rotations, including OB.
  • Participate in academic activities, including didactics, meetings, curriculum development and advising including one-half day didactic teaching and 3 half-days dedicated to scholarly work.
  • Provide coverage for FM inpatient services managing hospitalized patients at least 8 times per year.
  • Maintain accurate and timely documentation of patient care, adhering to all legal and regulatory standards.
  • Ensure high levels of proficiency within scope, following the expectations of the Patient-Centered Medical Home model.

This FQHC was established as a federally qualified health center (FQHC) in October 1999. As a FQHC, we receive some federal support to provide primary health care, oral health care and behavioral health care to anyone who enters our doors, without regard to socioeconomic barriers, including ability to pay. CCHC, a locally held, private not-for-profit, is governed by a volunteer Board of Directors that is required to maintain a consumer majority.

This FQHC offers a wide range of primary care options and focuses on a one stop shop philosophy to provide a caring environment for clinicians and patients.  Our comprehensive approach is complimented by bringing primary medical care, dental care, pharmacy, and testing together in one location.

How You’ll Help Patients:

  • Provide high quality medical care to the patients in accordance with Specialty Certification and policies and procedures.
